I, the parent or guardian, do hereby grant permission for my child to participate in sports and related activities with Grace Evangelical Church (GEC)/Crosscheck. I understand that due to the physical contact associated with this activity, there is a possibility that my child may sustain physical injury while participating. In order that my son/daughter may receive any necessary medical treatment he/she may need, I hereby authorize the coach or other supervising adults to obtain it, and hereby hold GEC/Crosscheck and its representatives harmless in the exercise of such authority. I further release GEC/Crosscheck, and its representatives from any claims for personal illness or injury that my child may sustain.
A concussion is a brain injury caused by a bump, blow, or jolt to the head, or by a blow to another part of the body with the force transmitted to the head. It can range from mild to severe and can disrupt the way the brain normally works. Even though most concussions are mild, all concussions are potentially serious and may result in complications including prolonged brain damage and death if not recognized and managed properly. Signs and symptoms of concussion may show up right after the injury or can take hours or days to fully appear. If your child reports any symptoms of concussion, or if you notice the symptoms or signs of concussion yourself, seek medical attention right away. I, hereby acknowledge having received education about the risk of sports related concussions as provided in the information above. I also acknowledge my responsibility to report to my coaches, parent(s)/guardian(s) any signs or symptoms of a concussion.
